What is an Advance Directive?
Advance Directives are legal documents that allow you to make your healthcare wishes known before a medical emergency occurs. It enables you to outline the types of medical treatment you would or would not want if you become unable to communicate your decisions. An advance directive may also allow you to designate a trusted family member or friend to make healthcare decisions on your behalf if you are unable to do so.
Having an advance directive helps ensure your preferences are respected and can provide guidance and peace of mind for your loved ones during difficult situations.
Advance Directive forms are linked below and information about these directives is available on the Tennessee Department of Health site. All forms have a section to write comments about circumstances in which you would not want CPR, a feeding tube, or various other scenarios. We encourage you to discuss your wishes with your healthcare provider and your loved ones before completing your advance directive.
