Who may Refer a Patient for Home Health Services?

Documentation Requirements:
- Referral Orders signed within 7-10 days
- Face to Face (within 30 days prior to referral)
- Plan of Care (485) signed with (7-10 days of admission)
- Verbal orders (signed within 30 days)
