Homebound individuals who are under the care of a physician requiring nursing or therapy services.

Home Health Services Are Covered by:

Who may Refer a Patient for Home Health Services?

  • Physicians
  • Hospitals
  • Nursing Homes
  • Rehab Centers
  • Dialysis Centers
  • Assisted Living
  • Insurance Companies

Documentation Requirements:

  • Referral Orders signed within 7-10 days
  • Face to Face (within 30 days prior to referral)
  • Plan of Care (485) signed with (7-10 days of admission)
  • Verbal orders (signed within 30 days)
